Will there be food service? (Required)

A $100 security deposit is required in addition to the “cooking charge” for guest food service providers to use the CUBC kitchen facilities and will be applied towards any food service-related incidents that result in Christian Unity Baptist Church losses, equipment damages, and/or costs to clean/restore the facilities of Christian Unity Baptist Church after the conclusion of your event. The security deposit will be refunded in full, if no CUBC items are damaged and/or removed from the CUBC facilities and the facilities used are restored to the satisfaction of the assigned CUBC representative. If food service-related damages exceed the $100 security deposit, the Responsible Person in this agreement agrees to pay in full for the restoration of the CUBC facilities related to this event. Tablecloths are not provided by Christian Unity Baptist Church for your event.

Company name and contact information Visiting food service providers (whether professional, family, or friend) must meet with the designated CUBC Coordinator prior to preparing/serving food of any type in Christian Unity Baptist Church. The food service provider assumes full responsibility for any and all food served and the actions of all helpers, volunteers, and/or persons assisting in preparing and providing food service for this event.
